Branch Recital Programs

To Apply for the Recitals

1.	You must be a member of MTAC Santa Clara.
2.	Submit an application for each student. You can:
	A. Click here to submit the form on-line.
	B. Click here to download a printable form and mail it.
	C. Photocopy the form from the yearbook and mail it.
3.	Send in a check written by the teacher for each student.
